The plan is active for one year from the date of purchase. Pricing is based on household size:
Individual: $159/year
+1 Person: $199/year
+2 People: $249/year
+3 People: $299/year

Members receive no-charge services such as:
One comprehensive exam (initial visit)
Two periodic exams (every six months)
One emergency exam
Routine X-rays
Oral ID cancer screening
In addition, all other dental services are discounted 15%, except for dental products.

Yes. The plan:
Is valid only at White Oak Dental.
Cannot be combined with other dental insurance or discount plans.
Is non-transferable and non-refundable.
Cannot be used for services outside the dentist’s capability, or for injuries covered by workers’ compensation or automobile medical coverage.
Requires patients with periodontal disease to complete deep cleanings before maintenance cleanings are covered.
